Transfer Out of State Commercial Driver License with Hazmat Endorsement

If you have moved to New Hampshire and need to transfer your out of state Commercial Driver License with a Hazmat Endorsement, you must request a Transfer Out of State License appointment. The following are requirements for all applicants:

Completed Commercial Driver License Application (DSMV 312)
Proof of Identity and Residency. Please review our Required Documents list for the type of license you wish to get: REAL ID compliant or non-compliant (not REAL ID).
Appropriate Fee
Vision Test
Valid Medical Certificate (unless exempt)
Fingerprints through TSA and your results must be back before you will be allowed to test.
Hazmat Knowledge test will be given (please review the CDL Manual)
