Duplicate Commercial Driver License
Lost or Stolen License
When you drive a motor vehicle in New Hampshire, you are required by law to have a driver license with you. If your license is lost, stolen or destroyed you must apply for a duplicate license.
Apply for a Duplicate License or Non Driver Identification
If you have lost or misplaced your Commercial Driver License, please make an appointment and bring the following information:
- Commercial Driver License Application (DSMV 312)
- Copy of current Medical Card (unless you are exempt)
- Appropriate Fee
Temporary Duplicate License by Mail
If you are temporarily away from New Hampshire and your license is lost or stolen, you may apply for a temporary duplicate license by mail. If your image is not on file at the DMV, you will be unable to get a duplicate license by mail. To apply, please send the following documents:
- A completed and signed application for Commercial Driver License.
- A Temporarily Away form providing an out-of-state address.
- Copy of current Medical Card (unless you are exempt)
- Appropriate Fee

The DMV will mail a temporary duplicate license which will be valid for six (6) months to your out-of-state address. Once you return to New Hampshire, you are required to submit a new Commercial Driver License application form through our Drop Box services, along with a note explaining that you have returned to the State and a photocopy of your temporary license, and a permanent license will be mailed to you.
