Gender Change

To change the gender on your license, you must request an appointment for a Gender Change. You will need to bring the following items to your appointment:

• A completed Application for Driver License (DSMV 450) or Application for Commercial Driver License (DSMV 312)
  • On the application, check the box for Replacement, and under Reason, please indicate “gender change.”
• Your current, valid NH Driver License or Non Driver Identification card.
• Payment of $3.00
