Requesting your own Motor Vehicle Record (any DMV Drop Box)

Please submit the following information in an envelope and place it in one of the Drop Box locations:

• Application for Motor Vehicle Records (DSMV 505)
• $15.00 fee (made payable to "State of NH-DMV")

Requesting someone else's Motor Vehicle Record

Please submit the following information in an envelope and place it in the Concord Drop Box only:

• Application for Motor Vehicle Records (DSMV 505)
• $15.00 fee (made payable to "State of NH-DMV")

Requesting an Accident Report

Please submit the following information in an envelope and place it in the Concord Drop Box only:

• Application for Motor Vehicle Records (DSMV 505)
• $5 minimum Each additional page over 5 pages $1 each, or a telephone number where you may be reached to take a credit card payment

Responding to a ticket

Please submit the following information in an envelope and place it in the Concord Drop Box only:

• Signed ticket with plea indicated
• Appropriate fee as indicated on ticket (made payable to "State of NH-DMV")

Restoration Fee

Please submit the following information in an envelope and place it in the Concord Drop Box only:

• Note indicating what payment is for, and include name and date of birth
• Appropriate fee depending on what type of restoration fee is being made (made payable to "State of NH-DMV")
