Driver Licensing Transactions

Non Driver ID Renewals

If you are not eligible to renew online, please include the following information in an envelope and place it in one of the Drop Box locations, and your new Non Driver Identification card will be mailed to your current address:

Completed and signed Application for Driver License or Non Driver ID (DSMV 450)
Photocopy of current Non Driver ID
$10.00 fee (made payable to "State of NH-DMV")

Duplicate Driver License/ Non Driver ID/Commercial Driver License

If you have lost or misplaced your Driver License or Non Driver ID, please include the following information in an envelope and place it in one of the Drop Box locations, and your duplicate Driver License or Non Driver Identification card will be mailed to your current address:

Completed and signed Application for Duplicate Driver License or Non Driver ID (DSMV 637)
$10.00 fee (made payable to "State of NH-DMV")

If you have lost or misplaced your Commercial Driver License, please include the following information in an envelope and place it in one of the Drop Box locations, and your duplicate Commercial Driver License will be mailed to your current address:

Completed and signed Application for a Commercial Driver License (DSMV 312)
Copy of current Medical Card (unless you are exempt)
$10.00 fee (made payable to "State of NH-DMV")

Motorcycle License Endorsement

If you have completed a motorcycle rider training course, please include the following information in an envelope and place it in one of the Drop Box locations, and your updated Driver License with your motorcycle endorsement will be mailed to your current address:

Completed and signed Application (DSMV 450 for Driver License, or DSMV 312 for a CDL)
Photocopy of current Driver License
Photocopy of Completion Card
$30.00 fee (made payable to "State of NH-DMV")

CDL Medical Card update

If you have a new CDL Medical Card that needs to be entered or updated, please include the following information in an envelope and place it in one of the Drop Box locations:

A completed CDL Medical Card and Certification form (DSMV 612)
A copy of your valid Medical Card completed by a physician
Photocopy of current Driver License

Address changes

If you have changed your address, please include the following information in an envelope and place it in one of the Drop Box locations:

Completed and signed Record Change Request (DSMV 30)
Photocopy of current Driver License or Non Driver ID
If you wish to get a new Driver License or Non Driver ID with the new address listed, please include a $3.00 fee (made payable to "State of NH-DMV"). If you only want your address changed in the DMV system, you do not need to include the fee.
