You may request your own Driving Record either online, by making an appointment, or by submitting the following information in an envelope and placing it in one of our Drop Box locations:

Obtaining Another Person's Driving Record

You may obtain another person's Driving Record by submitting the following information in an envelope and placing it in the Concord Drop Box ONLY:

  • Application for Motor Vehicle Records (DSMV 505)
    • The named individual in Step 3 will need to have their signature notarized.  If the individual's signature has not been notarized, then Step 5 "Intended Use of Information" must be completed.
  • $15.00 fee (made payable to "State of NH-DMV")

Obtaining Another Person's Driving Record - Second and Third Party Requests

If you are acting as an authorized agent of your employer or a company, a Certificate of Authority must accompany the completed Release of Motor Vehicle Records (DSMV 505) or one must be on file with the DMV. Please note that these requests may only be processed by utilizing our Drop Box at the Concord DMV office or you may mail the completed form, along with $15 payment to:
