Requesting your own Driving Record

You may request your own Driving Record either online or by submitting the following information in an envelope and placing it in one of our Drop Box locations:

Release of Motor Vehicle Records (DSMV 505)
$15.00 fee (made payable to "State of NH-DMV")

Releasing Your Driving Record to Another Individual or Entity

You may release your Driving Record to another individual or entity by submitting the following information in an envelope and placing it in the Concord Drop Box ONLY:

Release of Motor Vehicle Records (DSMV 505)
- You must complete Step 3 by filling out the appropriate information for the third party that will be receiving your record.
- Your signature in Step 4 must be notarized by a Notary Public/Justice of the Peace
$15.00 fee (made payable to "State of NH-DMV")
